Why a film-forming surgical solution for preoperative skin preparation ?

Surgical site infection (SSI), the most common nosocomial infection complicates operations performed even in developed countries. It results in to longer and costlier hospitalizations.

Recognizing the morbidity and economic burden, Centers for Disease Control  (CDC) issued guidelines for the use of antiseptic agents for skin preparation. This resulted in to the fall of incidence of surgical sepsis.

Why Alcohol+CHG based preoperative skin preparation ?

Isopropyl alcohol is the most effective broad-spectrum antimicrobial antiseptic agent. The combination of Isopropyl alcohol with Chlorhexidine gluconate have greater efficacy, easier applications, longer duration of action that is up to 6 hours and cost effectiveness, makes it popular preoperative skin preparation among surgeons, It has additional utility in certain urologic precedures. It's a one step preparation as opposed to a scrub-and-paint technique.

Advantages:

ToroprepTM has a fast acting, broad-spectrum antimicrobial activity and persistent antimicrobial activity up to 6-hours post-prep.

ToroprepTM contains two powerful antimicrobial agents:

Chlorhexidine gluconate disrupt the cell membranes of bacterial cells and Isopropyl alcohol also exhibits antiseptic properties, most likely resulting from protein denaturation. The synergistic effect of both the antimicrobial agents makes it a powerful skin prep product.

Both have long histories of use. No studies addressing the mutagenicity, carcinogenicity, or reproductive toxicity.

Chlorhexidine has practically no or minimally absorption through mature intact adult skin, except in a small numbers of neonates. Rare or no skin irritation in any clinical study.

Direction Of Use:

Completely wet the treatment area with Toroprep.

While removing applicator do not touch sponge. Discard the applicator after a single use. Not necessary to use the entire quantity available.

Use in well-ventilated area. Do not microwave or heat the solution applicator.

Apply to clean, completely dry, residue-free, incision sight. When hair removal is necessary and if a wet shave is used, thoroughly remove all soap residues.

Dry surgical sites. Use repeated back-and-forth strokes for 30 seconds. Use repeated back-and-forth strokes for 2 minutes for moist surgical sites (e.g., inguinal fold)

Do not allow solution to pool. Remove with tuck prep towels. Clean umbilicus with enclosed swabs. Avoid getting solution into hair. Hair may take up to 1 hour to dry. Wet hair could be flammable.

When prepping skin folds, toes, or fingers, use a sterile-gloved hand to hold skin apart until completely dry. Otherwise, skin may adhere to itself.

To reduce risk of fire,begin draping and/or using cautery only after solution is completely dry and all wet materials are removed.

Over the time tint will slowly fade away from the skin. Alcohol may be used to remove the tint if desired.

Composition:

Chlorhexidine gluconate IP 10% v/v

(equivalent to 2% w/v Chlorhexidine gluconate)

and Isopropyl alcohol 70% v/v

Precautions:

Keep away from open-flame. Do not use electrical appliances until complete drying of Torosept F takes  place (30seconds).

Keep away from immediate vicinity of eyes and do not use on mucous membranes.
